What you need for an Employment
- Job Application
- Resume
- A Current Negative Result of TB skin test or Chest X-ray result indicating negative TB. Please, call the Department of health for your TB skin test fee and request for testing or your clinic to request a TB shot
- Hepatitis B immunization or Declination form
- Social Security Card/ Permanent Green Card
- Criminal Background (Fingerprint) Check obtained from your Local Court/ Police Department
- If applying for CNA, your License must be current without a substantiated Finding in the North Carolina Registry Listing
All applicants must pass the criminal background check, and have no substantiated findings in the North Carolina Registry Listing to be considered for hire
